Hospice Social Worker

The Hospice Social Worker is a vital member of our interdisciplinary team, responsible for providing compassionate care and support to patients and their families. This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills.
Key Responsibilities- Provide preventive, educational, and evaluative services to meet the psychosocial needs of patients and their families.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Facilitate placement in long-term care facilities and monitor adjustment.
- Support and facilitate advance planning, including living wills, POA documents, and funeral arrangements.
- Perform other duties as assigned.
- Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or other field related to social work.
- One year of social work experience in a healthcare setting required; licensure if required by state rules.
- Master's degree in Social Work with one year of social work experience in a healthcare setting highly preferred.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Strong written and verbal communication skills.
- Ability to understand, read, write, and speak English.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
