Responsibilities:
In a fast-paced hospital setting, the RN Case Manager oversees, coordinates, and communicates all care and services provided for patients from admission through discharge. This role serves as the primary contact for families and patients regarding any care or length of stay concerns. The RN provides ongoing reviews of patient records based on established screening criteria approved by the medical staff. Additionally, prepares reports derived from the chart reviews and assists designated departments of the Medical Staff in implementing effective quality and utilization management systems.
Qualifications:
- Education:
High School Diploma, High School Equivalency (HSE), or completion of a CHS Approved Individualized Education Plan (IEP) Certificate is required.
- Licenses and Certifications:
RN - A current and valid Registered Nurse license to practice in the state of California is required.
BLS - Current Basic Life Support (BLS) certification for Healthcare Providers by the American Heart Association (AHA) is required.
