We are seeking a skilled Licensed Practical Nurse to join our team at Hope Community Resources, Inc. as a Skilled Care Specialist. In this role, you will provide high-quality patient care and support to our residents in a long-term care setting.
Key Responsibilities- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Assist with wound care, ostomy care, and other specialized patient needs.
- Monitor patient progress and report changes to the healthcare team.
- Participate in multidisciplinary planning and discharge planning to ensure seamless transitions of care.
- Current licensure as a Licensed Practical Nurse in the state of practice.
- One year of experience in long-term care or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Basic Life Support (BLS) certification.
- Experience in emergency or critical care environments.
- Comfort level working with vents, trachs, and PICC lines.
- Two years of experience in direct patient care.
