VNS Health is seeking a skilled Nurse Case Manager to join their team.
**Job Summary:**
We are looking for a dedicated and compassionate individual to coordinate care for our members.
**Key Responsibilities:**
* Assess member needs and identify solutions that promote high-quality and cost-effective healthcare services
* Collaborate with management, subject matter experts, physicians, members, and discharge planners to ensure timely and appropriate care
* Review requests for services and conduct clinical record reviews to ensure medical necessity
* Participate in case conferences and identify opportunities for alternative care options
* Maintain accurate records of care management and participate in approval for out-of-network services when necessary
**Requirements:**
* Current RN license
* Certified Case Manager preferred
* Associate's Degree in Nursing or a Master's degree in Occupational Therapy required
* Minimum two years of experience in case management or acute inpatient hospital care
* Knowledge of Medicare and Medicaid regulations required
* Experience working with community-based organizations preferred
