Occupational Therapist Job Description

We are seeking a skilled Occupational Therapist to join our team at Revival Home Health & Hospice, a part of BrightSpring Health Services. As an Occupational Therapist, you will play a vital role in providing high-quality patient care and making a meaningful impact in the lives of our patients and their families.

Responsibilities:
  • Assess and analyze patient care needs to establish a comprehensive plan of care, including discharge planning.
  • Assess the patient's environment to ensure safety and suitability for effective care.
  • Identify therapy diagnosis/patient care problems and interventions, making additions or changes as necessary.
  • Implement services and programs related to Occupational Therapy, ensuring orders are carried out in a professional manner.
  • Document additions and/or changes to the patient plan of care, re-evaluating progress and making changes as necessary.
  • Provide patient and caregiver education on diagnosis, medications, infection control, and safety measures.
  • Conduct comprehensive medication reviews and provide education on medication administration, actions, side effects, etc.
  • Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.
  • Complete and submit all required documentation within established agency guidelines.
Requirements:
  • Graduate of an Occupational Therapist Education program approved by ACOTE.
  • Current Occupational Therapist licensure in State and American Heart Association CPR certification required.
  • Two (2) years of experience in adult occupational therapy, home health experience preferred.
