As a key member of our clinical team, you will provide high-quality patient care, work collaboratively with our multidisciplinary team, and contribute to the development of our community health programs.
Key Responsibilities- Provide direct patient care, including triage, assessment, and treatment in accordance with established protocols and guidelines.
- Collaborate with our care team, providers, and community partners to ensure seamless patient care and coordination.
- Develop and implement patient education programs, including health promotion, disease prevention, and management of chronic conditions.
- Participate in quality improvement initiatives, including data analysis, process improvement, and policy development.
- Assist with the development and implementation of new programs and services, including outreach and community engagement.
- Current RN licensure in the state of New Jersey.
- Minimum of one year of experience in an ambulatory care or community health setting.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Knowledge of Microsoft Office, keyboarding, and experience with electronic medical records.
