Summary
A Hospice Nurse Practitioner plays a vital role in delivering advanced nursing care to patients and their families, while also serving as a clinical resource for staff and the interdisciplinary team.
As an independent practitioner, you will provide primary care, consultation, and expert advice.
Your duties will include making patient visits, being on-call, exercising prescriptive authority in collaboration with the supervising physician, and leading interdisciplinary team meetings.
You will manage care coordination for patients with complex medical conditions, as requested by the team. Conduct comprehensive assessments for these patients and oversee the care plan. Perform face-to-face patient encounters as needed, in accordance with collaborative agreements with physicians. Maintain accurate documentation of all services rendered, adhering to company-defined processes and forms. Provide clinical education internally and externally as required. Actively participate in team and staff meetings, serving as a clinical resource for all team members. Engage in quality and performance improvement initiatives as requested. Fulfill any additional duties as assigned.Education and Experience License Requirements Current CPR certification is mandatory. You must hold a valid certification as a clinical Nurse Specialist or Nurse Practitioner in your state of practice.
Current certification by ANCC or AANP is required, along with a valid driver's license, vehicle insurance, and access to reliable transportation.
Must possess an NPI Number, DEA with prescribing privileges, PECOS enrollment, CAQH enrollment, Medicare number, and preferably a Medicaid number (not required).
Experience of over one year as a Nurse Practitioner in chronic disease management, family, or primary care is essential. Additional State Practice Requirements for various states:No additional state-specific requirements.
