Job Summary

This role integrates healthcare for clients from pre-admission to post-discharge through coordination and sequencing of care. This integration enhances patient flow and promotes early intervention by interdisciplinary communication. The Healthcare Integration Specialist seeks to evaluate outcomes based on an integration among established clinical, financial, and utilization data.

This position functions in structured and unstructured healthcare settings described as a geographic and/or situational environment that may not have established policies and procedures. Utilizing independent nursing judgment when integrating healthcare, this role has knowledge of and utilizes appropriate age-specific structured care methodologies.

The ideal candidate will be able to work with growth and development needs of client populations in clinical areas, communicate effectively with patients, family members, and others on the healthcare team, and monitor, evaluate, and trend patient responses utilizing structured care methodologies.

The successful candidate will be fiscally responsible, controlling variances and duplication, and must demonstrate independence, assertiveness, and critical thinking when working with patients and coworkers.

We are seeking a highly motivated and organized individual who can prioritize tasks, adapt to changing situations, and work collaboratively as part of a multidisciplinary team. Additionally, this role requires strong problem-solving skills, self-motivation, and the ability to document clearly.

A valid driver's license may be required depending on location. The ideal candidate will also have strong flexibility and organizational skills, knowledge about reimbursement for services provided, and the ability to establish priorities.

Key Responsibilities:

  • Integrate healthcare services for clients from pre-admission to post-discharge
  • Coordinate and sequence care to enhance patient flow and promote early intervention
  • Evaluate outcomes based on integrated clinical, financial, and utilization data
  • Function in structured and unstructured healthcare settings, utilizing independent nursing judgment
  • Monitor, evaluate, and trend patient responses utilizing structured care methodologies

Requirements:

  • Bachelor's degree in nursing preferred
  • Graduate from a nationally accredited nursing program preferred
  • Minimum one to two years of professional nursing experience in a clinic care setting preferred
  • In-depth knowledge of specialty area preferred
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring
  • Obtains and subsequently maintains required department-specific competencies and certifications

Salary Range:$34.00 - $51.00 per hour
