As a Care Coordinator I, you'll play a vital role in providing comprehensive case management, mental health, and substance use services to individuals with a history of persistent mental illness and/or substance use.

The estimated salary for this position is $23.20 per hour, ranging from $21.38 to $24.59 per hour. A sign-on bonus of $2,000 is available for external candidates.

Job Description
  • Assists clients to locate, access, and utilize a full range of health care services, community support groups, and resources;
  • Establishes and maintains housing; accesses mental and medical care, rehabilitation programs, and drug/alcohol services; increases social contacts and engages in constructive activities;

You will be responsible for assessing clients' needs and functioning, presenting cases in treatment team meetings, and monitoring progress as needed. Acts as client advocate interfacing with housing and support agencies, including property owners, support groups, Social Security, Medicare/MediCal, etc.

Requirements
  • Fluency in Spanish required;
  • A minimum of two (2) years of related experience or a bachelor's degree is required;
  • Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients;
  • Current California driver's license, car insurance, and a driving record acceptable to the Agency's insurance carrier;

This role requires knowledge of all job-specific skills, including crisis intervention, multicultural, and socio-economic issues, and documentation standards. You should also have knowledge of Agency policies and procedures, HIPAA, policies, and documentation guidelines, and county, state, and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use.
