POSITION OVERVIEW:
The Integrated Case Management Specialist-Resident plays a crucial role within the patient-centered care team, focusing on the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy. This position is dedicated to addressing the comprehensive health care needs of individuals and families through effective communication and resource utilization, ensuring patient safety, quality of care, and cost-effective outcomes.
This role is essential in supporting patients who have undergone significant health events or diagnoses that necessitate intricate management strategies and extensive resource utilization to enhance health outcomes throughout the care continuum.
Services are provided to patients in both outpatient and inpatient settings.EDUCATION AND EXPERIENCE REQUIRED:
A Master's degree in Social Work from an accredited institution is mandatory.
Proficiency in assessing and identifying barriers while collaborating with care team members to address patients' social and clinical requirements is essential.
Demonstrated customer service and interpersonal skills, with the ability to engage effectively with all members of the patient's care team.
Strong analytical, problem-solving, and decision-making abilities are crucial.
A compassionate approach with the capability to advocate effectively for patients and their families is vital.
CERTIFICATIONS AND LICENSES REQUIRED:
Limited Licensed Social Worker (LLMSW) with a valid, unrestricted State of Michigan license is required.
Must achieve full licensure (Licensed Master Social Worker-LMSW) within 12 months of completing 4,000 supervised hours.
