Outreach Therapist

Community Health Resources is seeking a highly skilled and compassionate Outreach Therapist to join our team in Willimantic, CT. As a key member of our clinical staff, you will be responsible for providing high-quality, evidence-based therapy services to families in need.

The Outreach Therapist will work closely with families to assess and address their unique needs, providing individualized treatment plans and interventions to promote stability, safety, and permanence. This is a full-time position, requiring a flexible schedule to accommodate client needs, with a minimum of 40 hours per week.

• Provide assessment and treatment interventions consistent with fidelity to evidence-based models
• Perform home and community-based care
• Document ongoing client progress and outcomes
• Maintain collaborative relationships with family members and other service providers
• Provide crisis assessment and intervention to monitor status of clients in crisis
• Work with clients to develop appropriate discharge plans utilizing natural supports
• Responsible for maintaining client and staff records according to agency policy
• Participate in weekly supervision, consultation, and meetings to review client and program progress
• Attend all required trainings

• Masters degree or higher in mental health field
• Demonstrated in-depth knowledge and experience as a psychotherapist working with people with mental illness and/or serious emotional disorders
• Demonstrated training and experience in all aspects of mental health treatment, assessment, evaluation, and provision of individual, group, and family therapy
• Licensure/Certification/Registration: LCSW, LMFT, LPC, eligible
