Home Health Nurse Opportunity

BAYADA Home Health Care is seeking a compassionate and skilled Nurse Care Specialist to join our team in Worcester, Massachusetts. As a member of our home health care team, you will have the opportunity to provide one-on-one care to patients in their homes, making a meaningful difference in their lives.

About the Role:
• Provide comprehensive nursing care to patients in their homes, including administration of medication, wound care, treatments, and procedures
• Monitor patients' conditions, reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
