About the Role:
- Deliver comprehensive nursing care to terminally ill patients in their homes
- Serve as a case manager and coordinate ongoing care planning, coordination, and outcomes
Responsibilities:
- Administer nursing care and manage patient care coordination
- Supervise support staff and plan, evaluate, and document care according to Agency Policies and Procedures
Requirements:
- Holds a current applicable state(s) license as a Registered Professional Nurse
- BLS certification is required for this role
- Prior experience in a similar position is preferred
