Key Responsibilities
  • Evaluate patients' functional abilities and limitations, determining the need for intervention and establishing a plan of care and treatment goals.
  • Implement individualized treatment plans, considering patients' physical, social, spiritual, cultural, educational, and age-specific needs.
  • Document patient progress and treatment outcomes, ensuring accurate and timely reporting to physicians, nurses, and other healthcare professionals.
  • Communicate effectively with patients, families, caregivers, and interdisciplinary teams to ensure seamless care coordination.
  • Exhibit adaptability in assignments, taking initiative to assist in other patient care areas and accepting responsibility for all patient care needs.
Requirements
  • Graduation from an APTA-accredited school of Physical Therapy or equivalent.
  • Current Physical Therapy license within the State of practice.
  • BLS certification required.
  • Ability to become proficient with computer software applications commonly used in an automated patient care environment.
