Position Summary
In this role, you will operate under limited supervision to deliver an advanced level of comprehensive healthcare to a designated patient demographic in partnership with the medical team. You will adhere to established nursing and medical protocols, providing specific care instructions to nursing and support personnel.
- Conducts thorough medical assessments and evaluations, including diagnosis, treatment, follow-up, consultations, and health education tailored to the patient's age and condition.
- Collects patient histories and formulates care plans, ensuring thoroughness and accuracy in documentation.
- Requests, obtains, and interprets relevant diagnostic tests.
- Creates, orders, and documents effective treatment plans, including medication prescriptions based on a detailed review of health history and diagnostic findings.
- Communicates care plans with relevant healthcare providers, including collaborating physicians, primary care providers, nursing staff, and case managers.
- Engages with patients and their families to explain care plans, disease processes, new treatment strategies, and medication schedules.
- Delivers medical care to patients experiencing illness or injury in accordance with accepted care standards.
- Provides health education to patients and families as necessary and appropriate.
- Records medical findings and care provided in the patient's clinical documentation, including laboratory and imaging study results.
- Educates patients on health maintenance procedures and supplies educational resources as needed.
- Collaborates with physicians and other healthcare team members regarding patient health needs, outcomes, and the effectiveness of care plans.
- Initiates necessary communication with other healthcare providers as required.
- Ensures the confidentiality and security of all protected health information, using and disclosing only information essential for job performance.
- Master's degree with completion of an accredited Nurse Practitioner Program is required.
- Current state licensure as a Nurse Practitioner is mandatory.
- Current state licensure as a Registered Nurse is required.
- Current BLS certification or obtained prior to delivering direct patient care is essential.
- Experience is preferred but not mandatory.
- DEA and CDS eligibility is required.
- Demonstrates the knowledge and skills necessary to provide care suitable for the patient population served. Possesses the ability to assess data reflective of the patient's requirements relative to their specific needs across the lifespan.
- Exhibits highly effective verbal and written communication skills to interact with patients, families, departmental units, and medical and nursing staff on all critical matters. Proven effective interpersonal skills are essential.
- Maintains up-to-date hospital safety and other mandatory training.
- Ensures updated immunizations as recommended for the practice area.
Ensures patient safety through active participation in hospital, department, or unit safety initiatives.
- Takes corrective action to address observed risks to patient safety.
- Reports adverse events and near misses to the appropriate management authority.
- Identifies potential risks in processes, procedures, and devices, communicating these to relevant personnel.
