Responsibilities- Assess patients' physical abilities and develop personalized treatment plans to help them maintain, restore, and improve movement, activity, and function.
- Provide interventions, conduct reexaminations, and modify interventions as necessary to ensure optimal patient outcomes.
- Collaborate with other healthcare professionals to ensure comprehensive care and effective discharge planning.
- Current license to practice physical therapy in the state where you will be working.
- Graduate from an accredited physical therapy educational program.
- At least one year of experience in physical therapy, preferably in home health care.
- Current CPR certification.
