Aveanna Healthcare LLC is seeking a skilled and compassionate Private Duty Nurse RN to provide high-quality care to our patients in their homes. As a Private Duty Nurse RN, you will be responsible for implementing physician-prescribed plans of care, conducting patient assessments, and coordinating care with other healthcare professionals.

Key Responsibilities
  • Implement physician-prescribed plans of care to ensure optimal patient outcomes
  • Conduct thorough patient assessments to identify needs and develop individualized care plans
  • Coordinate care with other healthcare professionals, including physicians, therapists, and social workers
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document patient care and progress in a timely and accurate manner
  • Provide education and training to patients and their families on health promotion, disease prevention, and management of chronic conditions
  • Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each patient
Requirements
  • Active, unencumbered RN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferably in home health care)
  • Reliable transportation
