About the Role

We are seeking a dedicated Home Health Physical Therapy Assistant to join our team in Cleveland, Texas. As a vital member of our healthcare team, you will provide top-notch patient care and support to individuals in need.

Salary: $70,000-$80,000 per year

Responsibilities

The successful candidate will be responsible for implementing care plans, communicating with patients and their families, and providing education on basic home safety and medical equipment safety.

  • Implement care plans and communicate findings or changes to the Physical Therapist and physician
  • Provide education to patients and their families on basic home safety and medical equipment safety
Qualifications

To be eligible for this role, applicants must possess an Associate's degree from an accredited school of Physical Therapy and hold a valid license, registration, or permit to practice in the state of operation.
