Job Summary

We are looking for a dedicated and compassionate LPN to assist our Patient Care Manager in coordinating high-quality care to home health patients. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Responsibilities
  • Assist with phone calls from field staff, physician offices, referral sources, patients, and take messages and orders as needed.
  • Monitor the status of incoming faxes and report information to appropriate personnel.
  • Coordinate communication and faxes applicable documents to physician offices.
  • Monitor the status of pending labs, facilitate needed follow-up, and communicate results to physicians and other personnel, as appropriate.
  • Monitor the status of patients in the hospital to ensure adequate care coordination.
  • Assist the Patient Care Manager with managing various software system reports for effective care coordination and utilization management.
  • Assist with payer source authorizations, as needed.
  • Perform any required follow-up in response to health check calls or tele-monitoring alerts, ensuring adequate communication to team members, as applicable.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with the scope of practice, plan of care, and physician orders, and in coordination with other members of the healthcare team.
  • Complete LHC required learning courses and additional assignments, as requested by the Executive Director.
  • Assist the Patient Care Manager with clinician documentation review/audits to ensure complete, appropriate, and accurate submission of all documentation by field staff.
  • Adhere to and participate in the agency's Episode Management process.
Requirements
  • Current LPN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
