MDS Coordinator

About the Role:

American Medical Associates is seeking a skilled MDS Coordinator to join our team at a beautiful privately owned skilled nursing facility located in Ohio.

Key Responsibilities:

• Conduct and coordinate the development and completion of the resident assessment (MDS) to ensure accurate and comprehensive care planning.
• Maintain and periodically update written policies and procedures that implement MDS and care plan to ensure compliance and best practices.
• Assist residents in completing the care plan portion of their discharge plan to ensure a smooth transition.
• Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of newly admitted residents.
• Assist Facility directors and supervisors in scheduling resident assessment and care plan meetings to ensure timely and effective care planning.
• Assist in determining appropriate treatment and selecting activities and exercises based on medical and social history of residents.
• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews to ensure ongoing care planning and improvement.

Requirements:

• Current Ohio RN license
• Long term care experience
• Experience as a MDS Coordinator
• Knowledge of MDS 3.0
