Physical Therapist

We are seeking a skilled Physical Therapist to join our team at CommuniCare Rehab, a member of the CommuniCare Family of Companies. As a Physical Therapist, you will have the opportunity to make a direct impact on the lives of our residents through therapy.

- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- Valid or pending state physical therapy license
- Evaluate and review patient/resident medical charts, evaluations under physician order, and develop a treatment plan in accordance with patient needs
- Implement treatment plan through direct treatment and education of the plan of care to all team members, including providing necessary supervision for PTAs
- Attend and lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of the rehab team, as well as conducting staff/family training and education when appropriate
