Compassionate Hospice Caregiver

Position Overview:
The primary responsibility of the Hospice Aide is to deliver personal healthcare and supportive services to patients in their residences. This role is essential in ensuring a safe and hygienic environment while collaborating effectively with patients and their families. The Hospice Aide will also communicate observations and concerns to the supervising registered nurse.

Traditions Health is actively seeking a PRN Hospice Aide to enhance our Hospice Team.
Qualifications:
- Education: High School Diploma is preferred.
- Licensure: A valid driver's license is required.
- Experience: A minimum of one year in a Home Health Aide or Nursing Assistant role is preferred, ideally within a hospital, nursing home, or home health agency. Completion of a State-approved training and competency evaluation program is necessary.
- Skills: Proficiency in reading, writing, and verbally communicating clinical information is essential. A demonstrated commitment to the well-being of the ill and elderly is required. Current CPR certification is mandatory. Successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test is expected.
- Transportation: Reliable transportation and valid auto liability insurance are necessary. *Mileage reimbursement is available.
The Hospice Aide will work in patients' homes, facing various conditions that may include exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is required, along with some exposure to adverse weather conditions.
Physical and Mental Demands:
This position requires prolonged standing and walking, with the ability to lift up to 50 pounds and assist patients with mobility. The role may involve working under pressure to meet deadlines and address patient needs, necessitating quick decision-making and effective communication skills. Manual dexterity and coordination are essential for using medical equipment in the home.
Key Responsibilities:
- Deliver direct patient care under the supervision of the RN, following the Aide Plan of Care.
- Assist patients with safe transfers and mobility in accordance with Agency policies.
- Accurately report changes in patient condition and document care provided to ensure continuity.
- Adhere to infection control protocols and maintain a clean, safe environment.
- Foster respectful and supportive communication with patients, families, and colleagues.
- Ensure patient privacy and property are respected.
- Support patients with nutritional needs and hydration.
- Contribute to the efficient operation of the Agency and demonstrate effective time management.
- Perform treatments and procedures as per Agency guidelines.
- Commit to professional development by completing 12 hours of in-service training annually.
- Uphold the Agency's philosophy and policies to guarantee quality care.
