In this role, you will work closely with our multidisciplinary care team to provide high-quality patient care and education. Your responsibilities will include providing and documenting patient care, triaging patients according to established protocols, and acting as a liaison between patients, staff, and providers.

You will also be responsible for monitoring task buckets, responding to tasks in a timely manner, and following up on overdue orders/worklists. Additionally, you will maintain open lines of communication with the care team and Regional Management team, and adhere to all patient safety initiatives and infection control regulations.

To be successful in this role, you will need to have an active, unrestricted RN licensure in CT, a graduate degree from an accredited School of Nursing, and intermediate level of computer proficiency in Microsoft Office. Experience working with electronic health records (EHRs) and knowledge of medical terminology are also highly desirable.

The estimated annual salary for this role is approximately $62,400-$114,000, based on a 40-hour workweek and the specified hourly rate. This figure does not include additional forms of compensation or benefits that may be offered by the company.
