Family Nurse Practitioner

We are seeking a highly skilled Family Nurse Practitioner to join our team at Bronx Community Health Network (BCHN). As a Family Nurse Practitioner, you will be responsible for providing high-quality primary care to adult and pediatric patients and their families.
Key Responsibilities:- Provide primary medical care to patients and manage acute and chronic conditions.
- Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
- Perform assessment/data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy.
- Collaborate with physicians and other clinicians to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
- Responsible for maintaining confidentiality of all patients, clients, proprietary, and protected information.
- Participate in patient care conferences, professional staff conferences, and other appropriate professional activities related to his/her assigned by the CMO.
- Master's Degree graduate of an accredited Nurse Practitioner program
- New York State Licensed Certification and Licensure as a Nurse Practitioner
- Current certification in Basic Life Support (BLS) through the American Heart Association (AHA) or willing to obtain within
- Board Certified
- Current active DEA license
- Relevant experience strongly Preferred.
- Experience working in community-based health care settings is also preferred.
- Experience/An understanding of EMRs
- The ability to demonstrate teamwork and effective interpersonal relations skills
- Excellent written/oral communication and computer skills
- Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients.
Exact compensation based on skills; experience $125,000 – 140,000 annually
