Join Our Team as a Care Coordinator for Home Health Patients and contribute to the delivery of high-quality care to patients in their homes. As a Care Coordinator with Stonerise, your primary responsibility will be to oversee the planning and direction of patient care services. This includes developing working patient care plans, supervising paraprofessional nursing staff, and regularly evaluating patient needs to revise the care plan accordingly. Your work will involve administering medications and treatments as prescribed by physicians, providing health education to patients based on assessment and care plans, and identifying discharge planning needs as part of the care plan development.

In this role, you will act as a Case Manager when assigned by the Clinical Manager and assume responsibility for coordinating patient care for assigned caseloads. To excel in this position, you should possess strong communication skills, empathy, and the ability to maintain accurate records of patient interactions. A passion for delivering exceptional care while promoting independence and dignity in patients is essential. Additionally, you must be adept at adapting to changing situations and prioritizing tasks effectively.

The ideal candidate for this role will have an active RN license in the state where they provide patient care, a graduate degree from an accredited school of nursing, and one to two years of recent acute care experience. Active CPR certification is also required. We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience, as well as opportunities for advancement, tuition reimbursement, and a comprehensive benefits package including paid time off, insurance, 401(k), and employee assistance program.
