We are seeking a passionate Occupational Therapist to empower patients to participate in their activities of daily living.

As an OT, you will work alongside our growing department of therapists to develop strategies and treatment plans to support patient growth and ability to overcome challenges.

This position is per diem.

Job Functions:
  • Conduct regular screening of residents to determine the best intervention or treatment for each individual patient based on scope of practice.
  • Perform treatment plans to residents; follow short and long-term goals and methods to achieve goals based on the established plan of care by supervising therapists.
  • Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
  • Establish individual resident therapy schedules in cooperation with other team members.
  • Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
  • Perform all other duties as assigned and as required to effectively treat patients.
  • Comply with all CMS documentation standards of practice daily with point of care documentations as indicated.
Requirements:
  • Associate's degree in Occupational Therapy.
  • Assumes responsibility for ongoing continuing education and professional development.
  • Must have an active Occupational Therapy License in Wisconsin.
  • Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.
