POSITION SUMMARY
The Patient Care Coordinator oversees designated team members and organizes their daily functions. This includes managing interdisciplinary care plans, assigning patients and families to nursing personnel, and ensuring the delivery of suitable interventions and support for patients, families, and caregivers by assessing their physical, psychological, social, economic, and spiritual requirements. The role also involves participating as a leader within the interdisciplinary team.
QUALIFICATIONS
Education: Completion of an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
Licensure: Must hold a valid Registered Nurse (RN) license in Arkansas.
Experience: A minimum of three years of nursing experience in a hospital, home health agency, hospice, or a related field is required. Supervisory experience of at least one year is preferred.
Additional Requirements: A valid Arkansas driver's license, personal vehicle, and auto insurance are necessary. Flexibility in working hours may be required.
PRIMARY RESPONSIBILITIES
- Supervise and coordinate daily activities of assigned team members.
- Conduct performance evaluations for all assigned staff promptly.
- Assign patient care responsibilities to appropriate team members, including RNs, LPNs, CNAs, Social Workers, and Chaplains.
- Ensure that Supervisory Aide Visits are conducted at least every 14 days by an RN.
- Maintain an updated census within the assigned team and distribute it to relevant departments and employees.
- Ensure coverage of IDG team member caseloads during absences and may serve as a supervisor for another team when necessary.
- Assist in the coordination, development, and revision of the patient's Plan of Care as required.
- Gather data on physical, psychological, social, and spiritual factors influencing patient and family health status upon referral and request.
- Facilitate effective communication with patients, families, interdisciplinary team members, and other healthcare providers.
- Maintain accurate and up-to-date patient records reflecting problems, interventions, and goals.
- Promote education on hospice care for patients, families, and team members.
- Utilize equipment and supplies efficiently and order necessary supplies, medications, and equipment.
- Lead interdisciplinary team meetings, case conferences, and care planning sessions.
- Coordinate medical/nursing student rotations through Arkansas Hospice, Inc. and serve as a liaison for university instructors.
- Supervise CNAs, RNs, and LPNs in accordance with state health and federal regulations.
- Actively participate in the Quality Improvement Program, ensuring accurate charting by assigned team members.
- Serve on clinical committees as requested.
- Engage in community programs to provide education about Arkansas Hospice, Inc. services.
- Rotate clinical administrative on-call duties to ensure 24-hour support for direct care staff.
- Communicate with the Area Manager regarding any office-related questions or concerns.
- Act as RN in charge of the office in the absence of the Area Manager, reporting directly to the Program Director or VP of Clinical Services/Designee.
- Assist with new employee orientation and serve as a mentor to new staff.
- Implement an ongoing evaluation system for staff and patient care.
- Ensure adequate staffing levels are maintained, participating in staff selection and assignment.
- Conduct interviews for clinical staff positions as needed and recommend hiring, retention, and termination decisions to the Area Manager.
- Maintain strict confidentiality at all times.
