We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. Responsibilities
  • Provide physical therapy services, including assessment, evaluation, and treatment plans, to patients in their homes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
  • Update care plans regularly and submit completed visit notes as required.
  • Perform OASIS time point assessments and discharge planning process per Medicare criteria, policy, and procedure.
  • Attend staff meetings, team conferences, and educational in-services as required.
Requirements
  • A current, unencumbered State Physical Therapy License in the state of practice.
  • Current driver's license, good driving record, proof of insurance, and reliable mode of transportation.
  • One year experience as a home care professional Physical Therapist and competent in performing comprehensive assessment and OASIS-C preferred.
  • The ability to make sound professional clinical judgment and assess and document patient needs.
