- Provide direct patient care in a rehabilitation setting, including assessing, planning, implementing, and evaluating patient care plans.
- Collaborate with interdisciplinary teams to develop and implement patient care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients to identify their physical, emotional, and social needs.
- Develop and implement individualized care plans that address the patient's needs and goals.
- Provide education and support to patients and their families to promote health and wellness.

Requirements:
- Current RN licensure in the state of practice.
