The Clinical Nurse Specialist is responsible for delivering expert clinical practice and care coordination, ensuring a comprehensive understanding of the needs of patients, families, nursing staff, and the broader healthcare team. This role emphasizes the provision of safe, effective, and compassionate care, tailored to the unique needs of each patient and their family.

Key Responsibilities:

  • Conduct thorough patient health evaluations by analyzing various data sources and understanding their interconnections.
  • Develop personalized care plans based on assessment results and trends within patient demographics to ensure optimal outcomes.
  • Deliver all patient care with a focus on patient and family-centered approaches, utilizing knowledge and experience to address the needs of patients throughout their care journey.
  • Champion patients' rights, providing guidance and support while serving as a resource for resolving concerns.
  • Leverage interdisciplinary resources to achieve desired patient outcomes and facilitate healthcare transitions for patients and families.
  • Maintain accurate documentation of all patient care activities in accordance with established standards, assisting team members with documentation as needed.
  • Assess the effectiveness of care processes, anticipate patient variations, and adjust care plans accordingly.
  • Make informed clinical decisions based on experience, patient data, and intuition, while fostering the development of clinical judgment among team members.
  • Communicate effectively and collaborate with colleagues, demonstrating respect and understanding for other healthcare disciplines.
  • Support staff in identifying patients' educational needs and measuring the outcomes of patient education initiatives.
  • Provide leadership and mentorship to colleagues, guiding practice and delegating patient care responsibilities as necessary.
  • Utilize established standards and guidelines for care delivery, ensuring a cohesive approach that integrates direct patient communication, technology, and teamwork.
  • Engage in professional development by completing a minimum of six formal contact hours and maintaining a record of continuing education.
  • Contribute to the evaluation of clinical and quality outcomes, participating in performance improvement efforts.
  • May undertake additional responsibilities as assigned.

Qualifications:

  • A minimum of one year of clinical experience as a Registered Nurse in an acute care environment.
  • Bachelor of Science in Nursing is required; candidates with an Associate's Degree must obtain a BSN within two years of employment.
  • Current BLS for Healthcare Provider Certification from the American Heart Association is mandatory.
  • Must possess a valid RN license or be eligible to practice as an RN.
  • Strong proficiency in English, with excellent written and verbal communication skills.
  • Proficient computer skills are essential.
