Nurse Practitioner

At AdaptHealth LLC, we are seeking a skilled and compassionate Nurse, LPN to join our team. As a key member of our healthcare team, you will provide specialized nursing services to patients and caregivers in the comfort of their own homes or alternative settings.
Job Duties:- Ensure physician orders are followed and incorporated into patients' plans of care.
- Develop and revise plans of care based on physician orders, ensuring nursing orders are carried out in a professional manner.
- Continuously monitor patient outcomes and report changes to supervisors to prevent hospital readmission.
- Participate in effective communication among other clinicians providing care to the patient.
- Complete and submit all required documentation within established agency guidelines.
- Communicate patients' progress to supervisors and make changes as necessary under the physician's direction.
- Cover after-hours, weekend, and holiday on-call shifts to meet the branch's needs.
- Maintain confidentiality of all company and patient-related information.
- Associate degree in nursing or equivalent.
- Current and unrestricted LPN clinical license.
- Two (2) years of clinical nursing experience is required.
- Five (5) years of related HME experience is preferred.
- Valid and unrestricted driver's license.
