Job Description:
We are seeking a skilled Clinical Value Nurse Practitioner to join our team of dedicated healthcare professionals. As a key member of our Clinical Efficiency & Value team, you will play a crucial role in driving improvements in patient care, reducing costs, and enhancing our overall value proposition. Your expertise will help us streamline processes, reduce length of stay, and ensure seamless transitions of care. By focusing on evidence-based practices and collaborative relationships with stakeholders, you will contribute to our mission of delivering exceptional patient outcomes while maintaining operational efficiency.

Key Responsibilities:
* Lead implementation of departmental programs aimed at improving value-based care
* Support staff in understanding the rationale for evidence-based value-based care
* Provide regular updates on the status of value-based programs to department leaders, clinicians, and operational staff
* Identify patients eligible for value-based programs and provide direct and indirect clinical care services
* Collaborate with Inpatient Case Management and Inpatient Nursing to support hospital discharge
* Develop collaborative relationships with members of the Medical Staff to gain support for value-based programs

Required Skills and Qualifications:
To succeed in this role, you should possess a Master's degree in Nursing (MSN) and national certification as a Nurse Practitioner. A minimum of five years of clinical experience in the relevant specialty area, along with experience in developing and implementing clinical programs, is highly desirable. Additionally, you should have strong communication and interpersonal skills, with the ability to build effective relationships with multidisciplinary teams. Strong analytical and problem-solving skills, combined with a passion for quality improvement, are essential for success in this position.
