Jet Health, Inc is seeking a dedicated and detail-oriented Home Health Psychosocial Specialist to join our team in Santa Fe, NM and surrounding areas.

Estimated Salary: $65,000 - $85,000 per year

About the Job:

Job Description

As a Home Health Psychosocial Specialist, you will conduct comprehensive assessments of patients' psychosocial, financial, environmental, and community resource needs.

  • Collaborate with patients, clients, representatives (if any), and caregivers to develop and evaluate the plan of care.
  • Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
  • Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
Requirements

To excel in this role, you should possess:

  • A Master's or Doctoral Degree from an accredited school of Social Work.
  • Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
  • A minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
  • Strong knowledge of community resources and excellent interpersonal skills.
