The Occupational Therapist PRN role at HCA Midwest Healthcare at Home offers a unique opportunity to provide high-quality patient-centered care in a home health setting. As a key member of our team, you will be responsible for managing the complete process of occupational therapeutic care for assigned patients, in coordination with the physician and patient care team.
Key Responsibilities- Evaluate and treat patients to support their functional status and independence.
- Develop and implement individualized care plans, focusing on goal achievement and patient engagement.
- Provide skilled interaction care, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Prioritize caseload, determine daily staffing needs, and coordinate assignments/handoffs.
- Occupational Therapy (OT) licensure and certification.
- Valid driver's license.
- Bachelor's degree in Occupational Therapy.
