Job Responsibilities

  1. Deliver direct patient care as defined in the state Nurse Practice Act.
  2. Implement the plan of care initiated by the registered nurse.
  3. Provide accurate and timely documentation consistent with the plan of care.
  4. Assess and educate patients and family/caregivers on self-care techniques and information relevant to diagnosis and care plan.
  5. Participate in coordinating home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
  6. Use and prepare equipment and supplies effectively and efficiently, adhering to aseptic technique.
  7. Participate in personal and professional growth and development.
  8. Perform other duties assigned by the registered nurse.
  9. Provide healthcare instructions to patients as appropriate per assessment and care plan.

Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, preferably in community/home health or medical/surgical settings.
  • Current licensure as a licensed practical nurse or licensed vocational nurse in the state.
  • Possession and maintenance of valid CPR certification.
  • Reliable transportation, current driver's license, and appropriate automobile insurance.

Salary: $60,000 - $80,000 per year, depending on experience and qualifications.
