About the Role

We are seeking a skilled Home Health Clinical Care Coordinator to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in ensuring our patients receive the highest level of care and support in the comfort of their own homes.

As a Home Health Clinical Care Coordinator, you will be responsible for overseeing treatment planning, evaluating patient needs, and coordinating with a team of healthcare professionals to ensure optimal clinical outcomes. You will work closely with physicians, nurses, therapists, and other caregivers to develop and implement comprehensive care plans that meet the unique needs of each patient.

Salary Information

The estimated salary for this position is $75,000 - $95,000 per year, depending on experience and qualifications. This job is eligible for a bonus incentive plan, which may provide additional compensation based on individual performance and company goals.

Key Responsibilities
  1. Evaluating patient needs and developing comprehensive care plans in collaboration with the interdisciplinary care team.
  2. Coordinating with physicians, nurses, therapists, and other caregivers to ensure seamless communication and effective care delivery.
  3. Monitoring patient progress, identifying areas for improvement, and making adjustments to care plans as needed.
  4. Fostering strong relationships with patients, families, and caregivers to ensure high-quality care and exceptional customer service.
  5. Staying up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery.
Requirements

To be successful in this role, you will need:

  • A Bachelor of Science degree in Nursing (BSN) or a related field.
  • At least 2 years of experience in a clinical home care setting, preferably in a management or leadership role.
  • Current CPR certification and knowledge of state and federal home health agency regulations.
  • Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and caregivers.
  • Strong interpersonal skills, with the ability to build trust and rapport with patients, families, and caregivers.
