Key Responsibilities:

• Conduct initial assessments and evaluations to determine impairments, diagnoses, prognoses, and care plans for individuals with varying rehabilitation needs in diverse settings, including acute care and outpatient facilities.

• Deliver or oversee therapy interventions that align with the assessment results, ensuring patient involvement in the care planning and implementation process.

• Regularly reassess patients during their treatment to monitor progress and adjust care plans as necessary, including the decision to discontinue therapy when appropriate.

• Plan for patient discharge by evaluating the achievement of goals and expected outcomes, ensuring appropriate follow-up or referrals are made.

• Maintain clear communication and documentation regarding all aspects of patient management, including evaluations, treatment responses, and any changes in patient status, collaborating with the care team as needed.

• Supervise and guide the activities of rehabilitation aides and support staff.

• Manage patient schedules effectively to accommodate varying caseloads and report any scheduling concerns to management.

• Participate in team meetings to communicate patient progress and ensure comprehensive understanding of each case.

• Engage in professional development and performance improvement initiatives.

• Provide education and consultation to patients, healthcare professionals, and the community regarding the benefits and purposes of therapy.

Qualifications:

• A Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institution.

• A minimum of 2 years of experience, with at least 1 year in a preferred setting.

• Experience with neurological conditions is preferred.

• Current certification in Basic Life Support (BLS) from the American Heart Association is required.

Work Schedule:

Full-time, Monday through Friday, including two evenings.
