Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our team at North Star Nursing. As a Home Care LPN, you will be responsible for providing clinical nursing services to our clients in the comfort of their own homes.

Key Responsibilities
  1. Client Assessment and Care Planning: Conduct thorough assessments of clients to identify their unique needs and develop personalized care plans.
  2. Medication Management: Set up and manage medications for clients according to our medication management program.
  3. Nursing Visits and Direct Care: Provide high-quality nursing visits and direct care to clients in their homes, as directed by our Registered Nurses.
  4. Staff Supervision and Education: Supervise and educate staff members on proper care techniques and protocols.
  5. Communication and Documentation: Maintain accurate and detailed records of client interactions and care provided, and communicate effectively with families, responsible parties, and other healthcare providers.
Requirements
  • Education and Licensure: Graduate of a practical nursing program and licensed as a practical nurse in the State of Minnesota.
  • Experience: Experience in home care or assisted living is preferred.
  • Skills and Abilities: Ability to read, write, comprehend, and carry out directions and instructions; ability to function independently; good health status and emotional stability.
Working Conditions

This position involves working in a variety of settings, including clients' homes, and requires the ability to lift, carry, push, and pull objects up to 50 pounds. The position also involves exposure to infectious wastes, toxic chemicals, and needles/body fluids.
