About the Role:- Conduct new client intake assessments for admission to services
- Develop and implement individualized plans of care
- Perform regular home visits to assess client needs and provide education
- Collaborate with healthcare teams to ensure seamless care coordination
- Document all patient care actions and maintain accurate records
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as a SC Licensed Practical Nurse or Registered Nurse
- One year of prior professional nursing experience, homecare experience preferred
- Current CPR certification
- Valid driver's license, car insurance, and access to an automobile for home visits
