Job Summary:

As a skilled Licensed Practical Nurse (LPN), you will provide direct and individualized nursing care to older patients based on the application of scientific nursing principles. This role requires a strong understanding of geriatric syndromes and the ability to recognize and manage common issues affecting older adults.

Key Responsibilities:

• Provide direct and individualized nursing care to older patients
• Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepare and administer prescribed medications, recording vital signs and medical information
• Assess patients' ability to perform daily tasks and assist with basic needs such as dressing, eating, and bathing
• Recognize and manage geriatric syndromes, including cardiovascular, respiratory, gastrointestinal, urinary, hematological, musculoskeletal, endocrine, neurological, psychiatric/psychosocial, integumentary, sensory, and pain problems

Requirements:

• High school diploma or GED
• Graduate from an accredited college, university, or vocational nursing program approved by the appropriate State agency and accredited
• LPN licensure: active, current, unrestricted in Ohio
• BLS certification by the American Heart Association or the American Red Cross
