Job Summary:
Mount Sinai Hospital seeks a Registered Nurse with exceptional care management skills to join our team. As a Nurse Care Manager, you will be responsible for delivering high-quality patient care and ensuring seamless transitions throughout the hospital stay.

Responsibilities:
  • Develop and implement comprehensive care plans that address patients' physical, emotional, and social needs.
  • Collaborate with interdisciplinary teams to ensure continuity of care and effective communication.
  • Conduct thorough assessments, identify risk factors, and develop strategies to prevent complications and improve outcomes.
  • Foster open communication with patients, families, and caregivers to educate them about their care and involve them in decision-making processes.
  • Maintain accurate records and documentation to ensure compliance with regulatory requirements and quality standards.

Requirements:
To succeed as a Nurse Care Manager at Mount Sinai Hospital, you should possess the following qualifications:
  • CURRENT RN NYS License and registration certificate required.
  • Bachelor of Science with a major in Nursing preferred.
  • BLS and ACLS certification (American Heart Association) required.
  • Progressive care experience a plus.
  • Specialty Certification preferred.
  • Cardiac Telemetry experience preferred.
  • IV/Phlebotomy, EKG certification skills required.
  • Critical thinking and excellent communication skills required.
  • Documented previous experience or educational preparation to support credentials.
  • Relevant clinical competence in area of nursing practice.
  • Minimum 1 year telemetry Med/Surg experience in a hospital setting.

Salary:$108,000 - $140,000 annually, based on experience.
