Medical Director Services PC is seeking an experienced Nurse Practitioner to provide primary care services to residents in a skilled nursing facility in Syracuse. The ideal candidate will have a strong background in nursing home care and reliable transportation. New graduates are welcome to apply. The successful candidate will be responsible for assessing patient health, performing physical examinations, and collaborating with the medical attending and nursing home staff to provide high-quality care. The provider will also be responsible for building relationships with residents and families, educating them on evidence-based care, and formulating plans to meet their needs and desires. The provider will also be responsible for maintaining a safe and clean working environment, complying with federal, state, and local regulations, and ensuring that charting, documenting, and administration of care are up to date and performed in a timely and efficient manner. Requirements include being board certified and having experience in a hospital setting. Extensive training will be offered. We are looking for a provider who is committed to quality and customer service, and who is able to work effectively in a team-based care environment.
