Job Summary

As a skilled LPN, you will provide direct and indirect nursing care to patients in routine nursing situations, under the direction and supervision of a registered nurse or licensed physician. In complex situations, you will function under the direct supervision of the RN or licensed physician, contributing to patient assessment and the formulation, implementation, and monitoring of the care plan.

Key Responsibilities
  • Assessment: Assist in the assessment of patient/consumer need for care, communicating with RN/MD providers appropriately and in a timely manner. Collect data relevant to visit/admission, ongoing care as directed by standards/plan of care.
  • Plan of Care: Contribute pertinent information to the plan of care and assist in monitoring care. Deliver nursing care as determined by plan or patient needs, recommending modifications as needed.
  • Implementation: Provide nursing interventions safely and competently, reporting relevant findings to RN/MD in a timely manner. Follow through and implement the plan of care as directed, providing appropriate instruction to patients/families about routine health maintenance, normal/abnormal lab results, special procedures, and follow-up care.
  • Evaluation: Assist in evaluating patient/consumer response to care through communication and observation, recommending modifications in the plan of care to RN/MD/ARNP/PA based on patient/consumer response.
Requirements

To be successful in this role, you will need to have a strong understanding of the physiologic and psychological effects of normal aging, as well as effective communication skills with patients/consumers of all ages. You will also need to be able to work collaboratively with other healthcare team members, participating in patient/consumer orientation and educational efforts.
