Job Description
  • We are seeking a skilled Caregiver Support Specialist to join our team. In this role, you will work collaboratively with RN Case Managers, CNAs, Social Workers, and other key members of the patient's care team.

Responsibilities include:

Patient Assessments
  • Conduct thorough assessments of patients' physical, emotional, spiritual, environmental, and educational needs.
  • Gather data from patients, families, significant others, or other healthcare professionals.
Patient Visits
  • Respond to patients' needs or changes in condition requiring acute intervention.
  • Provide interventions, treatments, and pain management to meet patients' needs.
Plan of Care
  • Read, understand, update, and follow the Plan of Care (POC).
  • Document interventions, outcomes, and changes in patient condition in the electronic health record (EHR).

Skills and Qualifications

  • ADN required; BSN preferred.
  • Current, unrestricted Registered Nursing License in state of employment.
  • One-year nursing experience preferred.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
