Key Responsibilities:

  • Conduct comprehensive assessments to identify member needs and develop individualized plans.
  • Navigate community resources and services to support member goals and objectives.
  • Establish and maintain partnerships with local organizations and agencies to enhance service delivery.
  • Provide education and support to members on independent living skills, employment, and education.
  • Coordinate with healthcare providers to ensure seamless care transitions and medication management.
  • Document all interactions and interventions in member records.

Requirements:

To be successful in this role, you should have a high school diploma or equivalent, a minimum of 2 years of experience in a related field, and a valid driver's license. A degree in social work, psychology, or a related field is preferred.

Salary Range: $42,000 - $52,000 per year, depending on experience
