The Habilitation Coordinator plays a pivotal role in empowering individuals with developmental disabilities to live their best lives. This dynamic position involves collaborating with the people we support and their staff to create personalized support plans that foster growth and independence.
Key Responsibilities:
- Develop and implement individualized support plans that promote quality of life and community integration.
- Monitor progress and ensure plans are being effectively implemented.
- Collaborate with staff to provide training and support in delivering high-quality services.
- Conduct regular assessments to identify areas for improvement and develop strategies for growth.
Requirements:
- Bachelor's degree or associate degree in human services or a related field.
- One year of experience working with individuals with developmental disabilities is preferred.
- Valid NYS Driver's License with two years of experience required.
