Job Overview

We are seeking a skilled Nurse Practitioner to join our team at PsychoGeriatric Services, providing exceptional care to seniors in long-term care facilities. As a key member of our mental health team, you will work closely with our facility partners to deliver evidence-based psychiatric services.

Responsibilities
  • Conduct on-site psychiatric assessments and medication management for patients in skilled nursing, assisted living, and other long-term care settings.
  • Collaborate with facility staff, including administrators, nurses, and social workers, to develop and implement individualized care plans.
  • Maintain accurate records and adhere to established regulations, policies, and procedures while exercising initiative and independent judgment.

Requirements
  • Possess a passion for working with seniors and providing exceptional care.
  • Hold a state license as a nurse practitioner (required).
  • Have a specialty as psychiatric nurse practitioner (PMHNP) or adult-gerontology nurse practitioner (AGNP) (preferred).
  • Be eligible for a DEA license.
  • Able to stand for 20 minutes and lift 15 pounds.
