Responsibilities
The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Evaluate patient's functional status and physical therapy needs. Assist physician in developing therapy plan of care.
- Observe and report patient's response to treatment and any changes in condition to supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
Qualifications
Licensure Requirements
- Current Physical Therapy licensure in state of MA
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable transportation
