Job Description:

We are seeking a skilled RN IHSS Case Manager to join our team. The ideal candidate will have a strong background in home health care, a passion for serving both pediatric and adult clients, and the ability to collaborate effectively with our team of healthcare professionals.

Responsibilities:
  • Provide high-quality, compassionate care to clients in their homes, ensuring their safety, comfort, and well-being.
  • Collaborate with supervising RNs to develop and implement individualized care plans based on client needs and preferences.
  • Conduct regular client visits within associated branch counties, demonstrating reliability and flexibility in scheduling and travel.
  • Utilize Kantime software for accurate and timely documentation of client care activities and progress.
  • Maintain open communication with clients, families, and interdisciplinary team members to coordinate care and address any concerns or changes in condition.
Requirements:
  • Current RN license in the state of Colorado
  • Minimum of 2 years of experience in a home health care setting
  • Experience working with both pediatric and adult clients preferred
  • Reliable transportation and willingness to travel within associated branch counties
  • Ability to work collaboratively with supervising RNs and other team members
  • Familiarity with Kantime software is a plus
  • Knowledge of IHSS regulations and procedures is desirable
