Key Responsibilities:
- Evaluate patient progress and response to treatment for the age population served, as defined in the department's scope of service.
- Develop and implement individualized rehabilitation plans to promote patient independence and well-being.
- Collaborate with healthcare teams to ensure comprehensive care and effective treatment outcomes.
- Conduct regular assessments to identify patient needs and adjust treatment plans accordingly.
- Maintain accurate and detailed records of patient progress and treatment plans.
Requirements:
- Minimum of one year of experience in a medical rehabilitation setting.
- BS in Recreational Therapy required.
- Certification (CTRS) or eligibility for certification within six months of hire.
